How do you enroll your child in the Head Start Program?

• Call CVCOG Rural Head Start office at (325) 944-9666 or contact your local Head Start Center.

• Children must be three by September 1st.

• The following documents are required to enroll your child into the program:

1. Proof of current income (TANF, SSI, 3 consecutive check stubs, current W-2, 1040 or letter from employer stating hourly wage & hours worked per week)
2. Birth Certificate
3. Social Security Card
4. Immunization Record
5. Food Stamp Case Number (if applicable)
6. Proof of Medicaid, Chips or Private Insurance (if applicable)
